Pursuant to the mandate of the National Privacy Commission to administer and implement the provisions of Republic Act No. 10173 or Data Privacy Act of 2012, by signing below it signifies that I/we have read, understood all the provisions and agreed with its full implementation.

1. I/We understand that by applying in NSA Philippine Cadet Program I/we give permission to collect and share with the sponsoring company and school the pertinent original copy of documents (submitted in hardcopy or via online submission in website, email, text messaging, google form, or any other social media platform), use and further process information on personal circumstances and directory information such as but not limited to address, phone number, e-mail address, family data, academic information, medical record and other significant data.
2. I/We am/are aware that the personal and sensitive information gathered will be accessed and used by NSA Program personnel, principal/manning company and school that are involved in the application for admission to the program, campaign, monitoring of cadets and alumni.
3. I/We am/are apprehensive that posting and processing of personally identifiable information on a website, email, text messaging or any other social media platforms might likely to happen for campaign, application updates, and in communicating with cadets and alumni. The posted information for campaign will be permanent (or item 7 may apply) while application updates will be valid for the active school year of application.
4. I/We understand that the submitted copy of documents, either hardcopy or digital, will be stored in a secured storage based on retention period and proper disposal (shredding of hardcopy and permanent deletion of digital copy) will be applied. While posted information for active cadets and alumni will be stored for as long as necessary and shall be permanently deleted after retention period expires.
5. I/We am aware that the Cadet Program can remotely process my application for screening, remote online examinations, virtual orientation and company interview and I/we warrant participation in video conferences related to the process of my application which includes the use of Microsoft Teams, Zoom, Skype and other video conference platform preferred by company representatives. The same applications can be used by the Program for academics, training and document processing.
6. I/We also acknowledge and warrant that I/we have acquired the consent from all parties relevant to this consent and hold free and harmless and indemnify NSA Program from any complaint, suit or damages which any party may file or claim in relation to my consent.
7. I/We are informed that a letter of cancellation will be submitted to NSA Program should I/we rescind to this agreement and rescission will take effect upon receipt of the request of cancellation.
8. I/We allow to retain any of my information as mentioned in item 4, or until such time that I submit to NSA Cadet Program a written cancellation of this consent. I/we reserve my right that all personal information will no longer be used in any business transactions after a written cancellation have been submitted.
